HEALTH SERVICES FACILITATOR

 

DISTINGUISHED FEATURES OF THE CLASS: 

 

The work involves responsibility for facilitating enrollment in public insurance and health supportive services programs.  The incumbent promotes public health insurance and health supportive service programs, ensures completeness and accuracy of application and referral information and provides technical assistance to enrollees regarding specific program benefits.   The incumbent also serves as a coordinator for program administration with subcontractors, community agencies, representatives of insurance companies, and the County Department of Social Services to develop and update procedures for enrolling clients with routine and special health care needs.  The work is performed under direct supervision of a higher level administrator.  Does related work as required.

 

TYPICAL WORK ACTIVITIES:  (Illustrative only)

Screens and refers potential clients for public health insurance and health supportive services programs;

Ensures applications are complete and accurate and forwards them to Medicaid or appropriate insurance plan;

Enters enrollment data into a local and/or New York State Department of Health database;

Provides training and technical assistance to subcontracting agencies and other department staff;

Develops outreach materials to promote health supportive service and insurance programs to the public;

Implements policies and procedures with input from community agencies, insurance companies and the County Department of Social Services to enroll children and adults in Medicaid, health insurance programs, and health supportive services;

Attends New York State Department of Health training sessions regarding program implementation and participates in quality assurance activities;

Interviews and assists in completing application and enrollment forms for appropriate health supportive services programs;

May determine presumptive eligibility for Medicaid and WIC as authorized by regulation;

Maintains knowledge of State rules and regulations regarding a variety of health benefits available to the public;

Ensures compliance with federal and state rules and regulations for program areas;

Creates and maintains a variety of records and reports as required;

May perform quality assurance and auditing functions and plan new methods and procedures for more efficient operation of the program.

FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS:

Good knowledge of the federal and state laws and rules relative to the program;

Working knowledge of modern office terminology, procedures, equipment and business English;

Ability to communicate effectively, both orally and in writing;

Ability to establish and maintain effective working relationships with others;

Ability to operate a personal computer and utilize common office software programs;

Ability to prepare reports relative to program activities;

Ability to understand and empathize with the needs and concerns of others;

Ability to understand and interpret complex written material;

Ability to plan and schedule workload to maximize capacity;

Ability to deal effectively with the public;

Physical condition commensurate with the demands of the position.

                                                                       

MINIMUM QUALIFICATIONS:

Graduation from a regionally accredited college or university or one accredited by the New York State Board of Regents to grant an Associate’s degree or higher and two (2) years experience in a public or private health care program.

 

SPECIAL REQUIREMENT: 

 

Possession of a valid New York State driver’s license is required.  Failure to maintain a NYS driver’s license including driving privileges throughout employment will result in disciplinary action or dismissal.
