PUBLIC HEALTH HOSPITAL COORDINATOR

 

DISTINGUISHING FEATURES OF THE CLASS: 

 

The work involves specialized public health nursing activities in a hospital setting or skilled nursing facility.  This position is responsible for evaluating, planning and directing patient's referrals to Home Health Agency for continuity of care.  Serves as a team member.  The work is performed under the supervision of the Director of Patient Services according to established policies, procedures and guidelines with wide leeway allowed for the exercise of independent judgment in carrying out the details of the work.  Does related work as required.

 

TYPICAL WORK ACTIVITIES:  (Illustrative only)

Acts as liaison between Home Health Agency, hospital, nursing facilities and other health care providers to ensure quality and continuity of patient care and to interpret agency services;

Receives referrals on patients who are hospitalized or in skilled nursing facilities and who are in need of follow-up care;

Facilitates referrals to Home Health Agency;

Evaluates need and assesses information regarding the patient's home situation to ascertain adequacy to meet patient needs;

Secures and assesses information from medical records as well as physician and other health care professionals' orders;

Participates in conferences with health care professionals, social workers, patient and family to develop plans for community based care and provide in-service education programs;

Evaluates need for necessary equipment to provide adequate home care and ensures hospital discharge planner is aware of need;

Coordinates patient's discharge with hospital to Home Health Agency;

Ensures necessary orders are completed and insurance information obtained;

Participates in quality assurance activities for the home health agency;

Prepares and presents staff in-services;

Assesses clinical competencies of staff nurses;

Prepares and maintains a variety of records and reports related to the work;

Shares on-call coverage with supervisory nursing staff;

May participate in agency surveys, studies and public relations activities;

May obtain prior approval for home care services from insurance carriers.

FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS:

 Thorough knowledge of current public health nursing practices;

Good knowledge of the administrative organization of community facilities and hospitals;

Working knowledge of current trends in the fields of medicine, nursing and education and their implications for planning continuity of patient care;

Working knowledge of federal, state and local laws pertaining to the care of individuals in the home;

Working knowledge of applicable insurance reimbursement criteria;

Ability to assess patient's total psychosocial and medical needs;

Ability to establish and maintain cooperative working relationships;

Ability to communicate effectively;

Ability to perform duties in accordance with ANA C ode for Professional Nurses;

Physical condition commensurate with the demands of the position.

 

MINIMUM QUALIFICATIONS:

A Bachelor's degree in nursing from a regionally accredited or NYS registered four-year college or university and one year of full time paid experience in community or public health nursing.

 

SPECIAL REQUIREMENTS:

1.        Licensure and current registration to practice as a registered professional nurse in New York State.

2.   Possession of a valid New York State driver’s license is required.  Failure to maintain a NYS driver’s license including driving privileges   throughout employment will result in disciplinary action or dismissal.
